Trying to find the notebook layout document

I have been unable to find the document you mentioned that shows a sample notebook layout. Can you find it and let me know where it is or at least the name of it?

The Student Training Manual attached here has the layout, but its hard to follow. I've extracted the layout of my notebook and attached it as a new forum topic. HTH
